Description of Issue

Request for a new Tyler Form to be created for either DocOrigin or Custom ReadyForm

Cause

Request for a new Tyler Form to be created for either Munis v11.3 DocOrigin or initiate a New Custom ReadyForm template from an existing Standard ReadyForm template.



Resolution
  1. Create a case in the support portal and title the case RFS New Form Request

  2. A Request for Form Modification form will be sent, including instructions to fill out pages 1 & 3 or 5, as well as a request to include a matching mock-up and data file (and ReadyForm template zip file if applicable)

  3. A KiteWorks link will be sent for secure file upload

    1. Once the Request for Form Modification form is completed, upload this and any other supporting/requested documents to KiteWorks via the link that will be provided by a Tyler Forms technician (the link does expire after 30 days)

    2. Leave a note in your support portal case stating that forms have been uploaded and be sure to set the case status back to Open

  • Example data files for check forms can be used from a test or train environment, as long as they match the mock up

  • If currently using Munis 2019+ and DocOrigin, and have access to ReadyForms, it is highly suggested to use ReadyForms standard templates to create new the desired forms

  • Creating a Custom ReadyForms template will cause a loss of ability to configure the template through the ReadyForms Administration template configuration, meaning any future form changes will need to be submitted via the Request for Service process and may be billable
