Request for Service Form Modification

Tyler Forms DocOrigin form modification needed, i.e. adding or changing a data field, logo, adding text OR setting up archiving, emailing, adding printer distribution, setting up email delay.

Request for general form modification including i.e. adding or changing a data field, logo, and/or adding text, etc. OR a request for processing or configuration modification i.e. setting up archiving, emailing, adding printer distribution, setting up email delay for DocOrigin Tyler Forms.

1. Create a case in the support portal and title the case RFS Form Modification

2. A Request for Form Modification form will be sent, including instructions to fill out pages 1 & 2 or 3, as well as a request to include a matching mock-up and data file

3. A KiteWorks link will be sent for secure file upload

   1. Once the Request for Form Modification form is completed, upload this and any other supporting/requested documents to KiteWorks via the link that will be provided by a Tyler Forms technician (the link does expire after 30 days)

   2. Leave a note in your support portal case stating that forms have been uploaded and be sure to set the case status back to Open

• Example data files for check forms can be used from a test or train environment, as long as they match the mock up

• If there are multiple copies of the form, please provide and list all applicable form names that the changes should be made to

• For any email configuration changes you must provide a valid data file containing an email address to be used for the programming of and testing the change
