This error generally happens when the same transaction record is submitted into workflow multiple times at once

• Mass acknowledge all workflow notifications for users
• To dismiss certain notifications, simply have the workflow approvers go to the location where they get notifications (Email, Hub, Workflow Assistant) and have them dismiss the pending notification
