Getting an error when import Projection Employee Master records

I am getting the following error when importing new Employee Master records into a Salary Projection:

FORMS statement error number -8017.

SFMT: Invalid index used.

• Employee Master

Missing date of birth in the import file and template

Add a column for Date of Birth in the import file, and to the import template.  To update the template:

1. Access the salary projection (Payroll > Salary and Benefit Projections > Projection Start and Status)

2. Search for a valid projection Run number > click Change

3. Click on Employee Import

4. Click Custom Templates > select Employee Information Import > click OK

5. Click View next to the template > click Continue

6. Click on column header > select Date of Birth > click Save
