Description of Issue

When clicking the ReadyForms Delivery Definitions document model button it does not show options. We cannot create a ReadyForms Delivery Definition. Hitting the accept tab generates this error:

SQL statement error number -201 (-102).
[Microsoft][SQL Server Native Client 11.0][SQL Server]Incorrect syntax near ')'.
