When generating the files needed to create the 834 report, some dependents are receiving multiple records.

Employee Dependents records for the dependents have mismatched information.

To resolve this, ensure that the following fields match across all the dependent's records:

• First Name

• Middle Initial

• Last Name

• Suffix

• SSN

• Date of Birth

• Gender

• Relationship
