Cause

Request Type was restricted in Role, Registrants or Anonymous Access by Request Permissions

Resolution

Verify Request Type is not restricted in the following places by Request Permissions:

Administration > Organization > Roles
Administration > Citizen Engagement > Registrants
Administration > Citizen Engagement > Anonymous Access
