Please contact TCM Support for further assistance. A site administrator or any user with the permissions to Maintain Mappings can review the mappings provided from that program screen to determine if a custom mapping was created and misconfigured:

As a temporary work-around, please click the downward arrow next to the paperclip > Document Mappings > select the mapping you wish to view documents from and click View Documents or double-click to open TCM. This should display the images in the TCM Viewer as normal.
