Description of Issue

Unable to load Chart via Chart Builder, getting error message:

There was an issue executing the merge into Organizations -268 ISAM -2601

Additional information concerning a status error of -268:

Cannot insert duplicate key row in object 'dbo.Organizations' with unique index 'IX_Organization_Unique'. The duplicate key value is ().

Cause

Environment was upgraded to latest Service Pack (2019.1.12.0) that included fixes that were not deployed to QuickStart/COA Builder.  The file format is different from what the Service Pack allows.



Resolution

Updated the file to include missing columns on Fund Attributes worksheet for Service Pack 12.  Refresh was completed and file imported successfully.
