EE missing in positions 15-16 on ICMA report

I created my ICMA report for 457 deductions and the EE is missing in positions 15-16 on the report.

• Munis
• Payroll Start and Status
• Deduction Report

401/RHS Employer Contribution, 401/RHS Employee Mandatory Pre-tax, 401/RHS Employee Voluntary Pre-tax, 401/RHS Employee Mandatory After-tax, and IRA Deduction fields are not populated.

1. Access Payroll Start and Status: Payroll > Payroll Processing > Payroll Start and Status.
2. Click Deduction Report.
3. Click ICMA.
4. Define deduction ranges for 401/RHS Employer Contribution, 401/RHS Employee Mandatory Pre-tax, 401/RHS Employee Voluntary Pre-tax, 401/RHS Employee Mandatory After-tax, and IRA Deduction field.
5. Click Accept.
6. Click Save.
